This is the one page on this site that sends information to us. Everything else, the evaluator, the pathway assessment, runs entirely in your browser and transmits nothing. Using this form is a deliberate choice, and you can undo it whenever you like.

Before you fill this in

We will never ask you for a fee, a seed phrase, a private key, a password, or remote access. If anyone contacts you claiming to be from Chain Pursuit and asks for any of those, it is not us. Report it.

The form actively rejects wallet addresses and transaction hashes. Those belong in your police report, not our database, see the evidence checklist.

Nothing here is pre-ticked, and the second one is genuinely optional, declining it does not change how we respond to you.

You may contact me about this submission. Required, or we have no reason to hold your details. I have read the privacy policy. Required. You may share this with a vetted firm if one fits my case. Optional. We will tell you which firm before we share anything, and we do not work with firms that charge fees before recovering funds.

We keep submissions for twelve months, then delete them automatically. You will get a reference number you can use to delete yours immediately at any point.

How we are paid

Where we introduce someone to a third-party firm and they engage that firm, we may receive a fee from the firm. We never charge you anything. We do not introduce anyone to a firm that charges money before recovering funds, and we do not use your assessment result to decide pricing or to qualify you.

Our partner list is currently empty, because no firm has yet supplied the licence number, written fee schedule and outcome data our published standard requires. Until one does, ticking the third box has no effect beyond recording your preference.
